Yes, he cuts of her nose to spite her face. She can either call for help and be permanently scarred or she can kill herself with the sleeping pills.

It was a model, John Doe cut her nose off. In one hand he had the phone glued and in the other pills. She could either ring the police and live a deformed life or kill herself (vanity) with the pills.
