Last May, I renewed my contract with O2 & got 12 months for £10/month tariff (reduced from the £25/month tariff) in return for not taking a phone upgrade.

I called them up today & asked to pay off my remaining 3 months & get a PAC code. The woman said that ok but I will have to pay the remainder of the 3 months at the full tariff of £25/month.

This is extortionate given that I agreed to a deal of 12 months of £10/month yet they can charge me more if they want to.

Anyone else had problems with excessive fees for closing their account early?

Not really. That money off is a loyalty sub. It runs for the duration of the contract but once you choose to cancel you have to pay the original tariff price.

Otherwise whats to stop someone upgrading to a brand new phone, having their subs and then just cancelling? Thus saving a nice amount on the cost of the handset.
