I am getting a DD amp via mail order. Is it likely to include the lead that connects the DVD player to the amp? Cant remember what the lead is called but I think there are two types (not phono).
If not how much do they cost?

Assume your talking about the Optical link (TOS).
Never seen any connection cables included with an amp, there usually supplied with the sources (tape, tuner, etc). In anycase the ones supplied are pretty lame.
from my experience DVDs do not have inteconnects supplied except possible a scart. So for audio go out and buy a decent one

Arent there two types? How much do they cost?
Optical (TOS) and copper (co-ax). If you can (i.e. if your player and amp support it) you should go for co-ax - it's generally reckoned to "sound better". You will also get a better quality at a lower price. If you have to use optical, buy the best you can afford (but don't spend £1,000 even if you can afford it!)
co-ax - look to spend around £20.
optical - around £40.

Co-ax, as long as it's 75 ohm, any phono cable will do the job. It's also more resilient to damage than optical. Only go for optical if you don't have a co-ax output on your DVD. While you are all in the mood to help out newbies, I 've just acquired my first 'real' surround system (Videologic Digitheatre DTS and Toshiba SD110E) and have connected the Digitheatre to the DVD (successfully!) but was wondering how I go about getting TV played in surround sound. I have a Sanyo nicam video (18 months old) and a 5 year old JVC 25" Nicam TV (NO ext speaker terminals). I assume I have to connect the amp to the VCR and watch TV thro it's tuner, but how do I go about doing that? What connections do I need? Many thanks.

First, does your video have 2 phono out sockets for audio? If so, just get a pair of phono cables and connect them to the DTS. ailing that, do you have an AV2 scart socket on the TV or VCR? You may be able to get a Scart-2 phono cable and output your sound to the DTS that way.
... or your TV may have phono audio out.
