what is the difference?

from what i have read all of the extras are on the new version were on the R4?

there certainly is a difference. just depends whether they are things you care about. the three main changes are:

- there is now audio commentary from james cameron on all of the deleted scenes

- cameron is now also featured in the documentary

- the 'subtitle' issue has been fixed. the brief section of text that always appeared in one whole piece before the first shot of the future had been omitted, carved up into pieces and presented as subtitles during the first 20 seconds or so of that sequence. this caused quite a stink as it was such a noticeable and inferior change.

and you also get the original mono soundtrack, although I have heard that it is not as good as the mono soundtrack from the first OOP release, or did I dream that ??

After listening to the excellent DD 5.1 soundtrack, I can't say it would bother me!
